The 5-Point Gracewell Healthcare Personalised Care Programme

In a Gracewell care home, you are at the heart of everything we do. In the care home sector, this is called ‘Person-Centred Care.’

To us, it’s simply common sense to make sure the care we provide to older people takes each individual’s likes, dislikes, preferences and care needs into account. This is the basis of the standards that we set for ourselves.

We create a Personalised Care Programme based on these five criteria:

Your Personalised Suite

In your personalised suite, you will choose:

  • A range of bespoke wallpaper: from specific colours to printed images
  • An exclusive range of high quality pillow options
  • A hand-picked range of specialist comfortable chairs
  • A selection of wall-hanging art

Bedroom

Your Personal Interests, Social Life and Companionship

We’ll find out what you really enjoy doing - even old hobbies that you like to recall. Together, we’ll create a plan for activities and introduce you to other residents who share your interests.

Typical activities include visits to a badminton game, a Hollywood movie, star photograph exhibitions, music and dancing performances, shell-collecting, chess playing, knitting and craft club, or painting lessons…a whole range of ways to stay socially active and to have fun in your new home.

We also host the Tuesday Teapot Club each week: an opportunity for our residents to get to know each other a little better.

Your Favourite Meals and Dining Choices

Each and every one of our residents enjoys a highly varied range of delicious, healthy food that also supports their dietary needs. From the moment we start talking to each other, we’ll aim to find out about your personal food preferences so that we can include them in our menus.

Special dietary requirements are always considered and we always aim to ensure that our meals are both nutritious and appetising.

Your Personal Care, Nursing and Support Needs

In order to fully understand your specific nursing and support needs - before you move into your new home - we work closely with you, your family, General Practitioners and nurses to identify the level of care required. Each personal care plan seeks to provide each resident with a say in their care.

Your Family Updates, Visits and Activities

Our real reward is in seeing our residents living happy, full and independent lives. Very often, that’s simply a matter of enabling people to see their family and friends regularly and stay up to date with what’s going on in their lives.

We actively encourage friends and family to take part in the regular activities and events we hold at our care homes and to talk to us about their loved one.
