Quality Care Services for Older People

At Gracewell Healthcare, we create homes that people love to live in. Places that are full of life and activity. Places that our residents love to call home.

And the care? We’re professionals in the field of providing care for older people and take enormous pride in the quality of the care we provide.

Whether you refer people to us for residential care, dementia care, nursing care or a respite break, you can be confident that your patient is being taken care of by an experienced, highly trained team of carers, support staff and registered nurses.

Individual, Personalised Care

Every care plan in a Gracewell home is centred on each individual’s personal needs and tastes. So, before anyone moves in, we’ll talk to them, to their families and to you about the five-point Gracewell Personalised Care Plan. Our aim: to create a care plan that’s tailored to specific needs.

  1. Personal interests, social life, former working life and companionship
  2. Personalising the bedroom suite to suit individual tastes
  3. Favourite meals and dining choices
  4. Identifying, monitoring and evolving personal care, nursing and support needs
  5. Keeping in touch with family and friends
