Care that’s all about you

Every time you refer a patient to Gracewell, our teams complete a care plan, we create a personalised care experience for each of our residents, because everyone is unique and has their own special likes and needs; everyone knows how they would like to be cared for and treated and everyone’s needs can change. Before anyone moves in, we'll talk to them, to their families and you. Together, we create a care plan tailored to your patient's needs. This plan considers:


  • Personal interests, their social and former working life and companionship needs and likes.
  • Identifying, monitoring and evolving personal care, nursing and support needs.
  • Favourite meals and dining choices/requirements.
  • Personalising the bedroom suite to suit individual tastes.
  • Keeping in touch with family and friends.

Professional Nursing care

In homes that are registered for nursing, our skilled and fully qualified nurses are committed to maintaining the health and wellbeing of each resident,  while utilising state of the art CareConnect to ensure the very best in nursing care.

Our en-suite nursing care bedrooms are 50% larger than the care sector average, giving residents and staff ample room to move around. They feature a state-of-the-art profiling bed that can adjust to different heights and angles as needed. And we can modify the rooms for any specialist equipment, such as ceiling hoists.


Dementia care

We know that memory loss affects every resident differently, and our dementia care team values each individual and focuses on their abilities, preferences and needs. 

We focus on understanding each resident to provide person-centred care, comfort and security in a familiar and enabling environment.

And, because nutrition and hydration play such a vital part of a person's wellbeing, our chefs are happy to prepare specialised meals to meet the dietary needs of individuals. Find out more about our dementia care services here.


Residential care

At Gracewell, we offer help and support with activities that range from personal hygiene to support at mealtimes, taking medications, getting dressed, taking a bath or going to the bathroom. Residential carers are available 24-hours a day, and nurses are on call should an emergency arise.

Independent living

Independent living at Gracewell offers the best of both worlds. A comfortable home with all the modern conveniences where an individual can retain their independence and a secure environment with qualified staff on-hand should the need arise for more specialised care in the future. 

Independent living is available at our Camberley, Fareham and Sway homes.


Respite care

Respite Care is offered to older people if their regular carer is unavailable, or if the individual is recovering surgery or an illness. The level of care provided is bespoke and delivered to the same standard as our long-term residents receive.


Palliative care

Our care for those needing end-of-life care ensures a comfortable dignified environment with medical staff, hospices and other professionals on hand whenever needed. Our expert team liaise with hospices, GPs and medical staff as required, drawing together a specialist group of professionals to provide the right level of care and symptom management.


Our commitment to care

We work closely with Alzheimer's Society, Dementia Friends and Dementia Action Alliance across our Sunrise and Gracewell homes to raise funds, and support their ongoing ability to run by providing meeting spaces and general assistance.
